I have a lot of lighters, and the X-tend is the most reliable lighter I have ever owned. Plus it has a neat safety feature that won't let it light upside down. While that may sound silly, some lighters look pretty similar right side up and upside down, and my wife once burned her hand lighting a Xikar Executive the wrong end up.

I got fed up with wasting money on lighters that get battered about and then fail so I bought this for £3 off eBay

    Neither more than $3/ea.

BTW: I have a Xikar Executive, and found it pretty finnicky as well, the only one I would put my faith in to light every time that I own is their tabletop triple-flame Liga Privada lighter.
Love, Love, Love this cheep lighter! Probibly the same make/desighn as Steve's above. I've been though all the same issues with expensive lighters. I never could get myself to pay $200 for a lighter when that would buy a nice box of cigars. Anyways, why would you want to spend that on something that just creates fire? I have two lighters I use and have used for the last couple years with NO ISSUES!
The first is this lighter and the second is my Zippo lighter with a "Z-Plus Butane" insert.
However onto the topic I do know that Xikar will replace it for free as they do with any of there products. They have great customer service too.

I've yet to have a Xikar that won't light, now I've sent them back for cracked plastic or finish that wears down rather quickly, but they always light9If they've got fluid)! I've also got a few ronson, dealextreme and other cheap lighters. Love the dealextreme its got huge tank and for $1-$2 if it doesn't work no big deal toss it and grab another.

Hi. My name is Vic, and I'm a lighterholic. I can't help it. I just keep adding to the collection, and cheapies make it easy to feed my habit. So far, the most expensive lighter I have been able to bring myself to pay for is a duPont X-tend that I got on sale for $100. Although pricey, I have to admit that it's been worth every penny.

Update on my lighter situation: it appeared to be a striker problem. An over the counter swap solved it.
I'm not from Texas but I got here as fast as I could.
