A fingernail always works good to work a circle to cut away at the cap - I always try to just take the cap off & leave the filler uncut anyway (if I can)... though if it's a belicoso or torpedo, this doesn't work too well - only then will I resort to teeth.

If I have a knife I'll work a little circle on the cap using the tip of the blade and rotate the cigar to peel off the cap. It sounds a whole lot more technical than it is in reality... Whatever works...
